چکیده

In clinical practice, stress testing might be performed due to lack of typical ST segment elevation in Wellens’ syndrome to obtain more evidence of cardiac ischemia. However, increasing cardiac demand during stress testing may result in acute myocardial infraction, and even fatal dysrhythmia and death [5]. Therefore, it is essential to consider the coronary angiogram as the initial diagnostic modality instead of other conservative examinations in patients with ECG patterns, indicating a possibility of Wellens’ syndrome.

Omnious T-wave inversions: Wellens’ syndrome revisited

Wellens' syndrome is characterized by T-wave changes in electrocardiogram (EKG) during pain-free period in a patient with intermittent angina chest pain. It carries significant diagnostic and prognostic value because this syndrome represents a pre-infarction stage of coronary artery disease involving proximal left anterior descending (LAD) artery, which can subsequently lead to extensive anteri...

Acute Type A Dissection and Acute Inferior MI Complicating Aortic Coarctation: A Case Report

Aortic coarctation is a rare but dangerous condition. Presence of aortic dissection in addition to coarctation is significantly rarer and more life-threatening. Here, we present a case of acute inferior MI due to aortic dissection complicating a native aortic coarctation. A 17-year-old boy was admitted to our emergency department due to severe chest pain and loss of consciousness. His ele...

Atypical Presentation of Acute Coronary Syndrome and Importance of Wellens’ Syndrome

BACKGROUND Acute coronary syndrome (ACS) is a common and potentially life-threatening condition encountered in emergency departments. Despite its dreaded nature, nearly one-third of ACS present without chest pain and may mislead clinicians. Additionally, Wellens' syndrome is a pre-infarction stage of significant proximal left anterior descending (LAD) artery stenosis, which can lead to extensiv...

Antiarrhythmic drug treatment: need for continuous vigilance.

Any physician treating cardiac arrhythmias should be aware of a possible worsening of the arrhythmia (so-called arrhythmogenesis) after the start of antiarrhythmic drug treatment. To facilitate the recognition of that complication the type and possible mechanisms of arrhythmogenesis are reviewed. Recognition also requires an understanding of (a) the time of appearance of arrhythmogenic effects ...

Electrocardiographic manifestations of Wellens' syndrome.

Wellens' syndrome is a pattern of electrocardiographic T-wave changes associated with critical, proximal left anterior descending (LAD) artery stenosis. The syndrome is also referred to as LAD coronary T-wave syndrome. Syndrome criteria include T-wave changes plus a history of anginal chest pain without serum marker abnormalities; patients lack Q waves and significant ST-segment elevation; such...
